Two letters of reference from host organizations (schools, youth organizations or coalition members in good standing) are also required. Complete our application below and upload the required documents. You will receive an email confirming your submission.

After your application with required documents is reviewed and approved by an FHCC officer, you will receive an Invoice in your email inbox.
